New Patients

Dear New Patient,

We are very excited that you are going to be coming to us for your urologic care! We know that today everyone has a very busy schedule to keep and we appreciate the importance of your time. In order to expedite the speed with which you are seen, as well as to make your visit with us is as pleasant as possible, we are providing you with our “New Patient” information sheets and other details for you to address ahead of time. Please feel free to fax, email, mail or drop off the patient forms prior to your appointment. Doing so will help to speed things up for everyone.

Please come with copies of any previous films, labs and/or notes from prior physicians which might help us to address your urologic problem.

Please be ready to provide a urine sample upon your arrival to our office.

Please come with a list of your medications, including dosages to your appointment.

If you do need to cancel or re-schedule your appointment with us, please let us know at least 24 hours in advance. This helps us to be able to provide the appointment time to someone else who is waiting to be seen. We understand that at times, unforeseen circumstances can come into play, but your consideration to other patients’ time, as well as ours, is appreciated.

We hope overall you have an enjoyable visit with us, no matter the reason for which you are seeing us. Please let us know if there is anything we can do to make your visit a pleasant. We are looking forward to seeing you.
